Position Overview:
The Support Coordinator level 2 provides non-clinical care and practical support to individuals with a NDIS Plan, helping them coordinate and implement supports, participate in the community, and achieve their goals. Build capacity to manage own supports. Part of the role will involve networking to allow for further referrals and to build upon existing caseload. There is a high focus upon working with participant who experience psycho-social disabilities.
Key Responsibilities:
- Relationship Management: Build collaborative relationships with participants, mainstream services, informal supports, and NDIS service providers.
- Service Coordination: Assist participants in understanding and utilising their NDIS plans, coordinating a range of supports and services.
- Internal Collaboration: Work with the Support and Recovery team to develop service documentation and implement participant goals.
- Administration: Maintain client databases and accurate records of support provided.
- NDIS Requirements: Adhere to and fulfil NDIS reporting requirements and associated tasks.
- Networking: Attend relevant networking and other related events to build a referral base.
Qualifications and Experience:
- Education: Certificate IV in Mental Health Peer Work or Mental Health. Relevant qualifications in Social Work, Psychology, or Occupational Therapy are also considered.
- Experience: At least one year in Support Coordination or other case management settings. Preferred – mental health-related peer work, preferably in in-home or community settings.
- Licenses: Current Driver’s License, NDIS workers’ screening clearance, and NSW Working with Children Check where applicable.
- Certifications: First Aid and CPR certification, with trauma-informed care and/or recovery-oriented practices preferred.
Skills:
- NDIS Knowledge: Understand NDIS guidelines and effectively utilise packages.
- Client Engagement: Engage and work effectively with clients, families, and stakeholders.
- Coordination: Coordinate a variety of supports as outlined in NDIS plans.
- Communication: Advocate for clients, write reports, and maintain clear records.
- Interpersonal Skills: Demonstrate empathy, patience, and cultural competence.
- Organisational Skills: Manage time effectively and meet deadlines.
- Problem-Solving: Identify issues and implement solutions.
- IT Proficiency: Use Microsoft Office products and electronic health records.
- Mental health: Experience working with participant who have high degree of mental health and psycho-social disabilities
